 Continuum of Care (CoC) 

What is a Continuum of Care (CoC)?

HUD's definition: A CoC is a community plan to organize and deliver housing and services to meet the needs of people who are homeless as they move to stable housing and maximize self-sufficiency.  It includes action steps to end homelessness and prevent a return to homelessness.

Essentially, a COC is a group of service providers that work together to address homelessness through housing and support services.

What are the components of a CoC?

  • Outreach, intake, and assessment
  • Emergency Shelter (ES)
  • Transitional Housing (TH)
  • Permanent Supportive Housing (PSH)
  • Prevention and Rapid Re-housing

What are the primary roles of a CoC?

  1. Develop, implement , and monitor the CoC Plan
  2. Apply for HUD CoC funding
  3. Conduct the annual Point-In-Time Count

Proposed GVPCCC Restructure

A proposal to restructure the GVPCCC was presented at the GVPCCC General Meeting by the Task Force on Homelessness on Tuesday, July 19th.  Proposed changes in the GVPCCC structure was motivated by the passage of the HEARTH Act.  The current structure of the GVPCCC does not easily lend itself to the new requirements of the HEARTH Act and a change in structure will be voted on by the GVPCCC at the Oct 18th GVPCCC general meeting.  Attached are outlines of the current structure and proposed restructure.
